Microneedling uses controlled needle penetration to create microscopic skin injuries that initiate a wound-healing response. FDA-authorized devices have specific cleared indications, including improvement in the appearance of facial acne scars and facial wrinkles in adults 22 years and older. Important regulatory note
The FDA has not cleared microneedling devices specifically to deliver platelet-rich plasma, cosmetics, peptide solutions, vitamin solutions, drugs or other topical products into the skin. 1. Microneedling with PRP

PROFESSIONAL MEDICAL DESCRIPTION
Microneedling with platelet-rich plasma (PRP) combines controlled collagen-induction therapy with an autologous blood product prepared from the patient's own blood. A blood sample is centrifuged to concentrate platelets and plasma. PRP may be applied or administered according to the clinician's protocol and applicable regulations.
Microneedling produces controlled microinjuries that activate a healing cascade and support remodeling of collagen and other extracellular-matrix components. Platelets contain signaling proteins associated with tissue repair. Because PRP is autologous, it avoids exposure to donor-derived biologic material; this does not eliminate procedural, blood-draw or infection risks.

A 2022 meta-analysis of 14 controlled studies involving 472 patients found that microneedling plus PRP was associated with greater acne-scar improvement and higher satisfaction than microneedling without PRP. Severe erythema and edema were not statistically increased. Study protocols varied, and these findings do not guarantee an individual result. [3]

Patient preparation checklist
SUN EXPOSURE
Avoid tanning beds and intentional sun exposure before treatment. Do not proceed on sunburned, recently tanned or actively irritated skin. Notify the clinic if a sunburn, significant tan, rash or infection develops before your appointment. FDA patient guidance advises against treatment when a patient is tan or planning significant sun exposure during the two weeks after the procedure. [1]
SKIN-CARE PREPARATION
- Use broad-spectrum sun protection and protective clothing in the weeks leading up to treatment.
- Unless your treating clinician gives different instructions, pause retinoids, retinol, tretinoin and strong exfoliating acids for approximately 3-5 days before treatment.
- Avoid scrubs, chemical peels, waxing and other irritating procedures near the treatment area.
- Arrive with clean skin and no makeup, sunscreen, oils or facial products.
- Do not pick, squeeze or aggressively exfoliate the treatment area.
- Maintain normal hydration and eat before the appointment when a blood draw for PRP is planned, unless instructed otherwise.

Medication safety
Do not independently stop prescription anticoagulants, aspirin or other medically necessary medicines. Medication changes require direction from the prescribing clinician.
Microneedling Post-Care
Makeup, sun protection, skin care and activity
NO MAKEUP
Do not apply makeup for at least 24 hours. Wait 48 hours or longer if redness, pinpoint bleeding, weeping, significant irritation or sensitivity remains. Resume only when the skin surface is intact, using clean brushes, sponges and products. The American Academy of Dermatology advises waiting 24 hours before applying makeup. [2]
SUN PROTECTION
Avoid deliberate sun exposure and tanning for approximately two weeks. Minimize direct exposure during the first several days, use shade and a wide-brimmed hat, and resume a gentle broad-spectrum SPF 30 or higher when your clinician advises and the skin tolerates it. [1]
FIRST 24-48 HOURS
- Use only the gentle cleanser, moisturizer, mask or recovery products approved by your clinician.
- Avoid strenuous exercise, saunas, steam rooms, hot tubs and excessive heat for approximately 24-48 hours.
- Avoid swimming until the skin surface is closed and irritation has resolved.
- Keep hands, phones, pillowcases and towels clean around the treated area.
- Do not pick, scratch, scrub or peel flaking skin.
FOR APPROXIMATELY 3-7 DAYS OR UNTIL FULLY RECOVERED
- Avoid retinoids, glycolic acid, salicylic acid, exfoliating enzymes, benzoyl peroxide, fragranced products and alcohol-based products.
- Use lukewarm rather than hot water and avoid aggressive cleansing devices.
- Continue consistent sun protection to reduce the risk of post-inflammatory pigment changes.
- Follow individualized instructions if a deeper treatment, PRP, sensitive skin or another procedure was involved.
Expected Downtime & Recovery
Typical short-term responses after treatment
- First 24 hours:
Common Experience: Redness or darkening, warmth, tightness, tenderness, mild swelling and possible pinpoint bleeding.
Care Priority: No makeup. Gentle products only. Avoid heat, heavy exercise and touching. - Days 2-3:
Common Experience: Redness generally decreases; dryness, sensitivity or mild swelling may continue.

Most microneedling reactions improve within several days, although redness, peeling or sensitivity may last up to a week or longer after deeper treatment. FDA-listed risks include dryness, roughness, tightness, redness, itching, peeling, discomfort, burning, bruising, bleeding and crusting; less common risks include pigment changes, cold-sore reactivation and infection.
